According to Fleischner, the most common failures he has seen include “inadequacy of screening…for both presence of mental-health problems and suicidal ideality, lack of adequate mental-health staff to provide mental-health services, inadequacy of suicide-prevention protocols in many facilities, with regards to noticing possibilities of suicide and responding to it, underestimation of the mental-health impact of overuse of segregation, seclusion, isolation, [and] inadequacy of discharge planning to appropriate community-based mental health.” y7 watch
